New Student Enrollment Information

2017-2018 Enrollment forms for NEW students

To complete the enrollment process for the2017-2018  school year, complete all the online and paper forms listed below.

*Online forms must be completed before any paper forms can be processed by the school.*

Paper Forms

Download, print, complete, and return to Tim Bloomingdale, director of admissions. *Copies of these forms were also included in the registration packet mailing.

Course Selection forms (refer to Program of Studies for course descriptions, prerequisites, credits, etc.)

Skutt Catholic Parent Association Membership Form *This form is OPTIONAL and is NOT required to complete the enrollment process.

Other Requirements

• Provide a copy of your student’s birth certificate to the Admissions office.
• The $350 registration fee must be included when returning the paper forms.
