Returning Student Enrollment

This page is for students RETURNING to Skutt Catholic.  If you are NOT a student at Skutt Catholic for the upcoming school year please go to the New Student Enrollment page.

Enrollment forms for RETURNING students

To complete the enrollment process for the upcoming school year, complete all the online and paper forms listed below.  All enrollment forms must be completed each year.

Online Forms

• Re-enrollment Form
• Student Health Form
• Technology Acceptable Usage Policy (TAUP)

*Online forms must be completed before any paper forms can be processed by the school.*

Paper Forms

Download, print, complete, and return to the Guidance office.
*Copies of these forms were distributed to students in school.

• Course Selection forms (refer to Program of Studies for course descriptions, prerequisites, credits, etc.)
  • 10th grade
  • 11th grade
  • 12th grade
• iPad Usage Agreement Form (11th grades only)
• iPad BYOD Agreement Form (10th & 12th grades only)
• Tuition Payment Plan Election Form
• Skutt Catholic Parent Association Membership Form *This form is OPTIONAL and is NOT required to complete the enrollment process

Other Requirements

The $350 registration fee must be included when returning the paper forms.
